Billy Cayman is a former patient at St. Bonaventure Hospital.

Season 2[]
Billy comes into the hospital following an assault at the jail where he is serving time on drug charges. After learning that the dent in his forehead is causing Billy's bullying problems, Dr. Alex Park suggests fixing it while Dr. Shaun Murphy is against it as the procedure is only cosmetic and unnecessary. Billy later reveals that he got the dent as a child from abuse from his father, causing Shaun to unexpectedly empathize with him. During surgery to fix Billy's injuries, Shaun suggests using a small breast implant to fill in the gap.
Despite a complication, the surgery is a success and Billy is elated by Shaun's solution to his problem. Shaun feels that he made the wrong decision as Billy nearly died, but Park reveals that Billy had been planning suicide before Shaun helped him and thus Shaun saved his life.
