"Islands: Part Two" is the twelfth episode of Season 1 of The Good Doctor. It aired on January 15, 2018.
Synopsis[]
The twins suffer complications from their surgery forcing the team at San Jose Boneventure Hospital to make a life-changing decision. Meanwhile, Dr. Shaun Murphy returns to the hospital after his trip with Lea and decides he needs a more permanent change and gives Dr. Aaron Glassman his two weeks' notice.
Plot[]
Following the separation surgery, Jenny wakes up but Katie is stuck in a coma. Shaun returns to work but Neil isn't happy how he lied about where he was. Claire and Shaun do an angiogram on Katie trying to find out why she's in a coma. Claire asks Shaun about his road trip, to which Shaun says that he kissed Lea and that Doctor Glassman is mad at him. Glassman and Shaun talk about him running away, while Glassman asks if this trip with Lea helped. Shaun says yes and that he'd like to give two weeks notice.
Jenny has a bad experience about being alone, due to how she doesn't have her sister with her for the first time. Jared has also got a lawyer to try to get his job back. Shaun suggests a stent to help Katie and helps the surgeon, Dr. Jordan Ko, put the stent into Katie, but it doesn't work so they abort the surgery. They learn that Jenny needs a new heart and that Katie is a perfect match. Neil is against it since Katie is still technically alive, while proposing their plan to the mother of the twins. She agrees to the surgery so that Jenny can survive.
Neil writes Shaun a recommendation letter and signs it, while telling Glassman to do so as well. After evidence of star doctors that happen to be white not being fired for assault is uncovered, the hospital decides to settle concerning Jared's case. Claire talks to Dr. Glassman about Shaun, stating that Shaun thinks that Glassman's mad at him. She suggests he signs the letter since it shows that he believes in Shaun. The surgery to remove Katie's heart is about to go ahead but Katie suddenly stabilizes. The doctors believe that if they reattach the twins that they'll help heal each other, though Jenny doesn't want to use Katie as a life support machine.
Claire wants to put the twins back in the same room so that Jenny can see that Katie really is gone. Shaun helps Lea pack, while saying that he wants to have his own life which he controls. However, Lea realizes that he’s actually following her and tells him he still needs help. Jenny agrees to the surgery after realizing that wherever she goes that Katie will always be with her and it’s a success. Shaun and Dr. Glassman finally talk and Glassman gives Shaun the letter of recommendation. However, he promises he'll back off Shaun and let him make his own choices if he stays.
Surprisingly, Katie wakes up but the doctors don't know how. Nonetheless, the doctors work to try and help them. Jared is also compensated, but all he wants his job back (to his lawyer’s annoyance). Claire asks Shaun if the reason he's moving to Pennsylvania because of Lea. He says that she makes him happy, though Claire tells him that Lea isn’t the only one who can do that and she'll miss him. Unfortunately, Jenny and Katie have to be separated again because Jenny is killing Katie. Katie starts crying because Jenny's health suddenly deteriorates and dies when her heart has nothing left to give. Katie refuses to separate from Jenny but her mother comes in and convinces her to let go.
Jared also gets his job back, but at a cost of Andrews losing respect for him as he has misrepresented racism and set it back. Shaun gives the letter back to Dr. Glassman and decides to stay, while Jessica tells Neil that she wants to break up due to their differences about kids. Lea gives Shaun the baseball they packed earlier, telling him to bring it when he visits her in Hershey. Lea and Shaun hug, while Lea tells Shaun that he's a good doctor, kisses him and leaves. Shaun puts the baseball on his dresser right next to the picture of him and Steve.

- This is the last episode to credit Paige Spara as a guest star. From season 2 onwards, she is a series regular.
