Dr. Isabel Barnes is a doctor at San Jose St. Bonaventure Hospital. She was the wife of Dr. Marcus Andrews, who is a surgeon and board member at the hospital. She and Marcus met in 1992 or 1993. Barnes is a hardworking woman whose priorities were with her career which was supported by her husband. Due to her husband's and her work focused demeanor, and her husband's lack of control of his ego there is tension between Andrews and his wife on the topic of having kids. When she and her husband have trouble conceiving, her husband becomes upset when the therapist suggests it may be due to a fertility problem in him. At the end of season 4, it's revealed that she has decided to move out and end their marriage and has already begun a relationship with someone else for unspecified reasons.

In "Fractured," Shaun seeks love advice from Andrews as he has been happily married to Isabel for twenty-seven years while Glassman has been divorced once. Andrews states that he knew he wanted to be with Isabel the first time they spoke in freshman year biology class, a resolve that has never wavered for him. Andrews tries to convey to Shaun how he knew he loved Isabel because every time he was around her, he was excited and his heart still speed up every time he sees her, even if in not quite the same way. Getting somewhat emotional talking about his love for Isabel, Andrews leaves to go be at home with her.
Season 4[]
In "Frontline Part One," Andrews is sleeping in the garage in order to keep Isabel safe from COVID-19 exposure.
In "Frontline Part Two," Andrews decides to return to the house and spend time with his wife.
In "Venga," while in Guatemala, Andrews flirts with Nurse Morales, causing Lim to warn him to be careful due to his wife at home. Andrews reassures Lim that she has nothing to worry about.
In "Vamos," after Morales sees Andrews' wedding ring, he reveals that Isabel is moving out of the house and has already started seeing someone else. Andrews admits that he went to Guatemala for a few days in order to get his head on straight and try to determine if he wants to save his marriage or not. Although Morales believes that Andrews still wants to save his marriage, he later gives his wedding ring away in order to pay for much-needed medical supplies, symbolically giving up on his marriage with Isabel.
