"Not the Same" is the fourth episode of Season 4 of The Good Doctor. It is the sixtieth episode overall, and aired on November 23, 2020.
Synopsis[]
Dr. Morgan Reznick and Dr. Shaun Murphy's pregnant patient with twins is having pain and they are forced to grapple with a decision no one wants to make. Meanwhile, Shaun asks Lea to move back in despite her reservations.[2]
Plot[]
Shaun and Lea are doing yoga together, but stop once it's time for work. Lea offers for their session to continue when she next comes over, but Shaun tells her that she shouldn't come over anymore and just move back in. She says living together means a bigger commitment and more trust, which she doesn't think they have yet. Shaun gives her his banking password, then leaves the room. Meanwhile, Claire and Lim are playing Rocket Racer together, but it's interrupted when Claire gets a text from Morgan. As it turns out, she needs her to go in as soon as possible. They have a patient, Billy, that has ingrown hair that has become an abscess and requires a surgeon. Enrique and Asher are there, as Claire tells them to give Billy the VIP treatment.
Shaun and Morgan are treating Kenzie, a woman pregnant with twins. Shaun hasn't got his residents with him, feeling that they're a distraction (though Morgan sees otherwise) while Shaun takes note of Kenzie's soaring blood pressure and she has preeclampsia. Shaun works with Olivia and Jordan to insert a central line in Kenzie to help her as the fetal heart rate is falling in one of the babies; her body is trying to kill it as a result of antibodies from an abortion she had.
Enrique finds Asher whom Enrique asks about his family. Asher hasn't talked to them in years apart from one of his sisters, having refused to follow their rules. Enrique states that his father was a real estate agent and Enrique thought he himself would spend his life selling condoms. They're soon given lab results which show that Billy has cancer. Later, Andrews asks Shaun about his residents. Shaun sees that Jordan talks too much, while Olivia talks too little and doesn't know where the jugular vein is. Andrews refers to Olivia's resume, stating it is not a lack of knowledge and tells Shaun to push her. He also thinks Olivia doesn't like to be in the spotlight.
Shaun works with Olivia, who does well while they're informed that the sick twin is in severe distress. Meanwhile, Claire tells Billy that he has melanoma, while Morgan says they can get rid of all of it right away. Park walks in as Billy talks about his deceased wife, from whom he got his good attitude from her. At the same time, Olivia is in surgery with Andrews and Shaun when she makes a mistake with one of the instruments, which Andrews blames on Shaun who is the senior. Shaun gets distressed at this and how he was given preferential treatment over the other candidate who was rejected in favor of Shaun when he first came to the hospital. Glassman tries to tell him to help Olivia, but Shaun fears about being unfair to Jordan. Glassman tells Shaun to understand their needs and ask how they feel.
Billy is in surgery as Park is revealed to be in a medium set of scrubs after Morgan hid the large ones. Billy suffers an anaphylactic shock as the team try to give him a pericardiotomy. Shaun finds Lea doing yoga at work, which he tells her she can't do. However, she retorts how he's having issues with it but not them living together again as a couple. However, he says he still feels the same about her; happy. Shaun tries to talk to Olivia who states that Kenzie is doing well. Olivia admits to being terrified, remembering having been made fun of in school for a mistake she made and now it's someone's life at risk if it happens again. Shaun can relate, due to having been bullied for his autism, when Jordan reports that the ambiotic sac of the stronger twin ruptured.
With Billy's cancer having spread to his liver, the team try to figure out what to do. When Enrique and Asher offer suggestions for procedures that won't make Billy better, Claire is given the final say as it's her case. Meanwhile, Park attempts to be there for Morgan, seeing she's upset as she can't be a surgeon anymore, but she just calls him pathetic. Meanwhile, the team for Kenzie's case discusses the possibility of doing a c-section, though sadly one baby may not make it. Shaun says neither baby has to die and wants to start labor, deliver the stronger baby, stop it and allow the weaker baby to thrive.
Kenzie is given the options and reluctantly agrees. As Billy is back in surgery, Olivia goes to see Andrews and they hug; she is actually her niece. She tells him that Shaun is very supportive, to Andrews' surprise. Lea goes to see Shaun, telling him that one of the reasons she is in the relationship is because he is different and can't see herself with anyone else. She agrees to moving in with him, thinking it's inevitable even though she's not sure about they have enough commitment and trust (which Shaun is thrilled with).
Kenzie has a drip inserted into her as Morgan tells Shaun that Jordan has a problem with him and he needs to rein her in, just like how he fixed Olivia. Morgan soon sees Park who is about to leave the hospital, while he admits he is finalizing his divorce. He knows it was over for years, but now it is final and it hurts. She offers for him to stay in her place, it is cheaper than a motel, which he turns down. Meanwhile, Kenzie in labor as Shaun asks Jordan how she feels and why she has a problem with authority. She tells him that if she is going to screw up she needs to know that he has her back.
When Billy awakens, Claire tells him that he has an infection and it could spread. He tells her that he understands that he might die, but he has had worse days. The day after his wife died, he was on his way to get a gun to commit suicide when he helped some kids who had a flat tire. Then he told them there was a sale on tires at Costco and he heard his wife laugh, which stopped him from killing himself. The first of Kenzie's babies is delivered, while underdeveloped and put on a ventilator. Now, Shaun has to stop the labor. The medicine is not working, with Kenzie still in labor. Shaun tells her that they need to do a c-section, or both of them will die. Jordan tries to calm her down because she is getting very upset, she thinks her baby will die if she has a c-section. Contractions are stopping and they don't need to operate.
Shaun meets with Glassman and claims to be unable to teach. Glassman states that Shaun is wonderful, claiming that he has been learning from him since the first day they met. Claire and Park are in Billy's room as he wakes up and looks good. Claire thanks her applicants for helping save a life. Jordan and Olivia bring in Kenzie's baby, while she is still pregnant with the other one. At home, Morgan is eating dinner and watching TV when there is a knock on the door; Park with his suitcase.
Olivia is outside sitting on a bench, where Andrews asks her if she wants a ride. He tells her that he wants to be there for her. She says accepting his help makes him feel like garbage. She tells him that she loves him but she needs to make this work. Andrews gets up and walks away. Lea is unpacking her things, while Shaun has made dinner for the two. They both say it feels like home again. Shaun says it's not the same as before; it's better.
